Human Resources Administrative Assistant

Full Time Position

Mental Health (Detox) Technician (MHT)

This position is qualified per NAC 458.341, which includes passing an examination forcertification as a detoxification technician as required by NAC 458.345. The scope of practice for a detoxification technician includes screening for the safe withdrawal from alcohol and other drugs and, obtaining and monitoring vital signs of clients. As with the other positions, this role will include passing a criminal background check and drug test. This role will also be asked to assist Nursing with day-to-day clinical tasks and will perform duties, including administrative tasks, that are required to safely manage the milieu.
A person may qualify for this position with an equal combination of education and experience, and may operate under the guidance of those with the proper degree and/or licensure/ certification.

New Admissions:

  • Required documentation for each new patient of the CTC:
  • Assisting Intake and the Nurse with the assessment of the patient’s behavior and medical
    status.
  • Aid the patient in review and signing of the required admission documentation.
    Provide input into the Multidisciplinary Care Plan
  • Document in the Electronic Health Record, (EHR) once per shift on all assigned patients,
    as to their level of participation, activity, engagement, and any concerns that you have
    noted during the shift.
  •  

Communication:

  • It is expected that the MA/CNA/MHT staff will demonstrate professionalism by:
  • Report all critical changes to vital signs or behaviors immediately to the RN-on-duty;
  • Provide updates to Nurse, Case Managers and Clinicians when medical status, mental
    health status, or crisis appear to have changed, escalated or improved, at any point during the shift;
  • Responding to emails, as appropriate within 24 hours, however, if the email is urgent or
    an emergency, will respond within 1 hour.
    Responding to phone calls within 24 hours; however, if the phone call is urgent or an
    emergency, respond within 1 hour.
  • Participate in change-of-shift report given by the RN-on-duty and peers coming off of shift daily at the beginning and end of shift unless otherwise directed.
  • Communicate with the team information verbalized by the patients that relates to
    discharge plans including need for housing, transportation, PUF services, if the
    information appears to be counter to the established plan reported at change of shift.

Required Documentation:

  •  Each MA/CNA/MHT must manage their time to complete all necessary documentation
    prior to leaving the facility.
  •  Each patient must have a note per shift on status, while enrolled at the CTC.
  •  Templates are permissible, but they must be patient -specific for any type of note.
  •  Any incident reports must be completed within 4 hours of incident, at the CTC.

Required Meeting Attendance.

  •  Shift report

Monitor CTC Patients’ daily:

  •  Make sure patients go to their daily groups.
  •  Check- in with all patients during the shift.

Transfers and Discharges:

  •  Prepare for transfer:
  •  Assist the RN with all required documentation to be leaving with the patient;
  •  Notify patient, and family, as indicated.
  •  Gather belongings noted on their Personal Property Log, ask for patient signature receipt,
    (as possible), or have RN sign as witness if patient is incapacitated, and bag for transfer
    with the patient.
  •  Note in EHR confirming transfer and the Personal Belongings Log has been
  •  Notify the Case Manager via email that a transfer or discharge, has occurred.
  •  Arrange for transportation to receiving facility, as needed.
  •  Communicate with the Nurse when the patient is prepared for discharge.
  •  If needed, provide support to Nurse and Case Manager when discharging to New Hope housing.
  •  Modify Rounds Board, and Census log.

Shifts: Day, Swing and Graveyard

Registered Nurse

This position is to aid in improving the quality of patient care, promote respect and dignity, and to ensure satisfaction for all patients in our clinic. The successful candidate will be accountable for supporting the Providers with the duties related to the care of patients in conjunction with the Medical Assistants.

Under the supervision of the MD and staff APRN, the RN position is an integral part of the multidisciplinary team and is the backbone of the services provided to those entering the Well Care Clinic for Primary and Behavioral Health Care needs.
  • Assisting the Providers with medical care to patients seen in the Clinic.
  • Administering immunizations and, medications under the Providers’ orders.
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable,
    and patient-centered
  • Documentation preparation, charting and assessment of patient care needs in
    collaboration with the Providers.
  • Act as an advocate and point of contact for clients treated at the clinic.
  • Ensure all documentation including lab results, imaging or other vital information
    is available for the Providers.
  • Adhere to the professional standards as outlined by protocols, rules and
    regulations while operating within the Scope of Practice.

Behavioral Health Technician/Transport Driver

Medical Assisting

We are seeking Certified Medical Assistants to become a part of our WC Health team! You will perform routine clinical and supportive assignments working with our team of Providers serving patients in our medical clinic setting. The position is Monday – Friday, no weekends or evenings.

  • Manage administrative duties as a part of patient care services provided, including locating relevant test results, taking vital signs, and ensuring the Providers have what is needed to see each patient.
  • Perform routine clinical tasks such as UA’s, COVID tests, and other Point of Care testing to support the services ordered by the Providers.

Payroll Administrator

Chronic Care Coordinator

Chronic Care Coordinator (CCC) works under the Chronic Care Management (CCM) program for the WC Health, Homebase Care or Alliance Mental Health. The CCC will collaborate with all care team members to ensure that patients enrolled in the CCM program receive the attention and care they need to meet their care goals and manage their chronic conditions. The CCC will communicate directly with the patients by phone or the CCM platform. They will provide education, care coordination, follow up and appointment assistance. The CCC works closely with the Medical Assistant to coordinate the care plan needs and follow ups.

  • Complete individualized patient centered care plans and provide chronic care management services using our CCM software.
