LAS VEGAS – (BUSINESS WIRE) – Amerigroup Nevada and MedTrans LLC dba WELL CARE Services have partnered to offer Medicaid members with chronic health problems, tailored integrated healthcare services at WELL CARE Services’ new facility in Las Vegas. The services are available to adults, and include onsite face-to-face primary care and telehealth exams, housing coordination, transportation to community resources and social services as well as therapy for mental illness and medication management.

As Medicaid membership increases in Nevada, so has the influx of Medicaid members visiting emergency rooms for chronic conditions. Access to mental health treatment or physical health care can be a challenge, especially for those who don’t have easy access to transportation and have complicated housing or health care needs. WELL CARE Services’ new facility offers a patient-centered model that helps members who are impacted by social and economic challenges during critical times of their care. Additionally, on-site behavior health case managers will increase face-to-face engagement by identifying and addressing factors that are impeding management of their health.

Through this joint-venture, WELL CARE Services’ field case managers will also work with behavioral health hospitals in Clark County to assist members with finding the right medical help at this new facility, and two other Well Care Services facilities in Las Vegas. For example, if an Amerigroup member who is homeless, and has lost all personal identification including medical identification card, walks into a local hospital’s emergency room for care, a referral will be sent to a trained mobile assessment team. A rapid-response mobile team will then assess the Amerigroup member’s needs and coordinate transportation for care to this new facility, or one of the other two WELL CARE Services facilities also located in Las Vegas. Each WELL CARE facility is staffed by high-skilled and licensed healthcare professionals, including a primary care physician, psychiatrist and case managers, who will provide intensive hands-on support and walk side-by-side with Amerigroup’s members to ensure that they get access to the appropriate level of care and community services to support their recovery and ultimately improve their health.

The new WELL CARE Services facility, serving Amerigroup members, is located at 5412 Boulder Highway in Las Vegas and open from 9:00 a.m. to 5:00 p.m., Monday through Friday.
